Patients must read and abided to all terms and conditions of the policy:
-All customers agrees to Maximum Pharmacy Courier-Services terms and conditions while on the company website, including or authorizing a drop off/delivery by the website or the companies business line.
- All customers agree: by booking online or by calling Maximum Pharmacy Courier- Services for a delivery, pharmacy pick ups, items or services you are authorizing and consent for Maximum Pharmacy Courier-Services to utilize your HIPAA information needed accordingly including authorization to call about your medication at the intended pick up destination only to verify that it is ready for pick up and to pick up your medication with the information provided by you( Name, DOB and HIPAA information) needed accordingly in order to provide direct timely delivery for you.
- Maximum Pharmacy Courier-Services will drop off locally to customers. Please call to confirm if your delivery locationis is considered a local area(s) for drop off at (854) 903-6787.
⁃ All Maximum Pharmacy Courier- Services employees will deliver only in well lit areas.
⁃ All medications will be return back to the originating pharmacy if the delivery is unable to be completed.
- We are not responsible if deliveries are not completed due to a discrepancy of but not limited to; location, cost, patient’s availability.
- We reserve the right to deny delivery in extenuating circumstances at ownership discretion.
⁃ Maximum Pharmacy Courier-Services is not responsible for any items once transactions are completed; patients are responsible for all prescriptions not received, damaged or stolen when requesting drop off in mailboxes or other unsecured package delivery boxes.
⁃ Maximum Pharmacy Courier Services is not responsible for any miscounted medications/ prescriptions.
⁃ Maximum Pharmacy Courier Services is not responsible for replacing any medications.
⁃ Maximum Pharmacy Courier Services is not responsible for the price of any item received by the patient.
- The patient is 100 percent responsible for giving Maximum Pharmacy Courier-Services the correct Pick up information (Pharmacy name, location, and phone number).
-The patient is 100 percent responsible for making sure the medication is ready(completes filled) ready for pick up for the Maximum Pharmacy Courier-Services employee.
Employees will not wait on any medication to be filled. All medications must be filled and ready for pick up before the employee arrives to the Pharmacy of pick up.
- The patient is 100 percent responsible for abiding to and paying for any prescription medications that is owed to the dispensing Pharmacy.
⁃ Patient must be at the location requested for delivery at the designated and agreed upon time unless otherwise approved by our ownership.
⁃ It is the patient’s responsibility to be present in order to receive all medications.
- Upon delivery a signature, confirmation of name and DOB are required for verification purposes. No prescriptions will be given without proper verification.
- Maximum Pharmacy Courier-Services employees may delivery to patients in company owned vehicles or personal vehicles in company uniform and badge. The name badge of the employee is the main requirement upon delivery by the employee.
⁃ No refunds or C.O.D.s (cash on delivery) accepted upon delivery.
C2-C5 ( controlled medications) will not be eligible for delivery at this given time.
- Upon using the payment methods listed - debit cards, credit cards, visa, master cards, and all other card formats may require an additional fee(s)...( processing fees) upon check out.
- Maximum Pharmacy Courier-Services reserves the right to change policy and terms of conditions accordingly.
Maximum Pharmacy Courier-Services is not responsible for any adverse reactions, interactions and allergic reactions of items purchased from Maximum Pharmacy Courier-Services of third party.
Our Site is protected by intellectual property laws. By agreeing to our terms and policies, the customer understands these guidelines. Maximum Pharmacy Courier -Services reserves the right to deny services at any time.
